A Patient-Centric Healthcare Framework Reference Architecture for Better Semantic Interoperability based on Blockchain, Cloud, and IoT


The application-centric perspective gives rise to the distributed and non-integrated nature of the healthcare systems, which in turn makes it difficult to manage the exchange of healthcare data (heterogeneity problem). On the other hand, Blockchain technologies are emerging as a promising and cost-effective means to meet some of these requirements due to the inherent design properties that they possess, such as secure cryptography and a resilient peer-to-peer network. This is due to the fact that Blockchain technologies are based on a decentralized ledger that is distributed across a network of computers. Similarly, applications that are based on Blockchain technology have the potential to benefit the healthcare industry due to their properties of asset sharing and audit trails of data access. The majority of the focus of the work done so far has been on centralized and Blockchain-based mechanisms. However, it does not take into account the growing demand for improved data interoperability across a variety of healthcare systems and services. In order to accomplish this, it is necessary to shift away from application-centric solutions and toward patient-centric solutions. This paper presents a secure and efficient framework for improving the interoperability of healthcare systems that is based on Blockchain technology, the cloud, and Internet of Things (IoT). It is called the Patient-Centric Healthcare Framework (PCH). To make the practical implementation of PCH possible, a tiered-based architecture with five tiers and collaboration has been designed. In addition, the design and implementation aspects begin with the layering diagram, the context of the system, and the detailed reference architecture. This architecture places an emphasis on the component topology and interactions that take place within the framework. An electronic medical record is used to demonstrate how data pertaining to healthcare is processed while taking into consideration the necessary safety measures. Following that, an analysis of PCH is performed in comparison to previously established Blockchain-based healthcare frameworks. The analysis of the results demonstrates that PCH provides practical solutions to protect data related to healthcare and support efficient data sharing with improved interoperability.
